DEMOLITION PERMIT- BASIC REQUIREMENTS

  1. Duly accomplished DEMOLITION PERMIT  form, signed by the owner/ applicant, signed and sealed by the architect/ engineer, form available at the Office of the Building Official.
  2. Photocopy of the updated PTR/ PRC of the professional
  3. Certified True Copy of the Transfer Certificate of Title from the Registry of Deed.
  4. 3 sets BILL of MATERIALS, notarized
  5. Photocopy of valid ID of the owner (preferably government-issued ID)
  6. Notarized Authorization Letter to transact in behalf of the owner or SPA from the owner
  7. Barangay Clearance for Demolition ,(if within a barangay)
  8. Homeowners’s Clearance for Demolition from HOA, (if inside a subdivision)
  9. Construction Safety and Health Program (CSHP) for Demolition, from DOLE
  10. Construction Business Permit from BPLO, Contractor’s Business Permit
  11. Tax Declaration from Assessor’s Office
  12. Real Property Tax Clearance (current year) from Treasury or Latest Tax Receipt
  13. One long folder (white)
  14. Photo of the building
